At the California Dental Association (CDA) protecting the privacy and security of your personal information and insuring accuracy of such information is very important to us. CDA does not sell any of your personal information to nonaffiliated third parties, although we do disclose personal information as described below in order to service your account. Accordingly, we want you to understand what information we collect and how it is used.
Your Nonpublic Personal Information
We collect and maintain "nonpublic personal information" in order to provide CDA members with a broad range of products and services as effectively and conveniently as possible. "Nonpublic personal information" is nonpublic information about you that we obtain in connection with providing insurance products or services for you and you and/or your family members. In certain circumstances, and only as permitted by law, we share such information with a select group of others in order to maintain or service your membership. This Notice of Privacy Policy describes our policies and practices with regard to collecting and disclosing such nonpublic personal information.
Confidentiality and Security of Your Nonpublic Personal Information
We restrict access to your nonpublic personal information to only those people who need to know such information to provide services to you. We also maintain appropriate physical, electronic, and procedural safeguards that comply with federal standards to guard your nonpublic personal information and protect against disclosure.
We do not sell information about you and we require any person or entity providing products or services to you on our behalf to protect the confidentiality of such information.
Categories of Information That We Collect
We collect nonpublic personal information we believe is necessary to administer our business, to advise you about products and services and to assist you in obtaining the insurance products and services you desire. We gather such information about you from the following sources:
- Information that you provide on applications, other insurance forms and written correspondence;
- Information we receive from you during telephone interviews; and
- Information from insurance companies that previously obtained it from you.

We do not disclose any nonpublic personal information about you or about former customers, except as permitted by law. We do disclose nonpublic personal information about you to affiliated and nonaffiliated third parties as permitted by law.
